Create a contact page

Checkmate comes with a contact page template that holds a contact form, so customers can write to you from your store. You create the page in your Shopify admin, give it the contact template, and then change the form in the editor.

Where messages go

Each message from the form is sent to your store's sender email address. You can change that address on the Notifications settings page of your Shopify admin, as Shopify's guide to contact pages describes.

Create the page

In your Shopify admin, add a page under Online Store > Pages, such as "Contact us". In the page's Online store section, select contact under Theme template, then save the page. Shopify's guides to creating pages and applying a template have each step.

The page shows its title as the heading. What you write in the page's Content shows under it, above the form.

Set up the form

The form always has these fields:

Field

Required

Name

No

Phone

No. Shows when Show phone field is on, as it is in the contact template.

Email

Yes

Message

No

There is no setting to add other fields or to rename these ones.

  1. In your Shopify admin, go to Online Store.

  2. Next to Checkmate, click Edit theme.

  3. At the top of the editor, select Pages, then contact.

  4. In the Page section, change Description. The sample text promises a reply within one working day, so change it to what you can promise, or delete it.

  5. In the editor sidebar, select the Contact form section.

  6. Optional. Turn off Show phone field to hide the phone field.

  7. Optional. In Submit label, enter the button's text. It reads "Send" by default.

  8. Click Save.

Show phone field and Submit label are at the end of the Contact form section's settings, below Style, Heading and Description.

After a customer sends the form, the form is replaced by a thank-you line. To write your own, enter it in Success message. Left empty, it reads "Thanks for reaching out. We will get back to you as soon as we can."

Add details beside the form

The Contact form section can hold blocks, such as a Details block for your email address, phone number, address and opening hours, an Image, Social links, a Text or a Button. Click Add block in the section to add one. It can also hold app blocks, as Add an app to your store explains.

Style decides where the section's Heading, Description and blocks sit:

Style

Where they sit

Centered

Above the form, centered. The default.

Side by side

To the left of the form on screens 1,000 px wide or wider, where they stay in view while customers scroll down a long form. Above the form on smaller screens.

Over background

To the left of the form on wide screens, with both over the section's background photo. Choose the photo in Background image.

With no heading, description or block, the form is centered whatever the style. The Contact form section describes every setting.

Questions under the form

In the Checkmate style, the contact template also has an FAQ section under the form, with five questions about the demo's coffee. Replace them with your own, as Create an FAQ page describes for the faq template, or select the section and click Remove section.

To show the page in your store's menu, add a link to it as Set up your store menus describes.
