The Contact form section shows a form that customers fill in to send you a message. A heading, a short paragraph and blocks, such as your email address, opening hours or a photo, sit above the form or beside it. To add one, click Add section in the editor and select one of the three Contact form presets. It works on any page, and Create a contact page shows how to give it a page of its own.
The form has fields for a name, an email address and a message, and an optional phone field. Only the email address is required, and its field is marked with a red star. After a customer sends the form, a thank-you message replaces it. Shopify sends each message to your store's sender email address, as Shopify's guide to contact pages explains.

What it does
Style
Centered puts the heading, the paragraph and the blocks above the form, in one narrow column. Side by side puts them to the left of the form, and they stay in view while customers scroll down a long form. Over background puts them to the left of the form, level with its middle, for a section with a background photo. On screens narrower than 1,000 px, every style shows them above the form.
Heading
Leave it empty when a heading above the section already names the page, as on Checkmate's contact page.
Description
A short paragraph under the heading.
Show phone field
Adds a phone field to the right of the name field, or under it on screens narrower than 600 px. Customers can leave it empty.
Submit label
The text on the button that sends the form. The default is "Send".
Success message
The message that replaces the form after it is sent. Leave it empty to show "Thanks for reaching out. We will get back to you as soon as we can."
If the heading, the paragraph and the blocks are all empty, the form shows alone in the center.

Blocks
Blocks sit under the heading and the paragraph. Click Add block in the section to add one. The Contact form with details preset uses these three:
An Image block for a photo. Here the photo is always cropped to a wide shape.
A Details block for labeled lines, such as your email address, phone number, address and opening hours, each with an optional link.
A Social links block for icons that link to your profiles in Social media settings.
Any other theme block works here too, and so do blocks from apps, as Add an app to your store explains.
